The St. Louis City Recorder and Registrar uses the Missouri Health Department's Vital Records Database to search for and produce computer-generated Missouri Certificates of Death dated 1980 to Present for any Missouri county. Service subject to:
- Availability of Missouri Vital Records Database
- Completion and accuracy of Copy Application by customer
- Death Record Restrictions
- Death Certificate Fees and Payment Method

Mail-In Customer
As of 02.28.2011, the Missouri Department of Health requires notarization of Applicant's signature (Customer signature) on the Death Certificate Copy Application when the Application is mailed in. The St. Louis City Recorder and Registrar's previous requirement for mailing a copy of Photo Identification with the Application is no longer applicable.

Walk-In Customer
As of 02.28.2011, the Missouri Department of Health requires Applicant's Photo Identification be shown before a Death Certificate copy is issued to a walk-in customer. The St. Louis City Recorder and Registrar has always required Applicants to show Photo Identification before Birth Certificate copy issuance.
